conducted a Risk Assessment and Toolbox talks for enclosed space entry. Making sure that a Permit to Work for enclosed space entry was issued and approved.
As the Safety Sea-Man with his team approached the enclosed space where the tank was located, he immediately noticed that despite the potential hazards present within the enclosed space, it was alarming to discover that there was no rescue equipment readily available at the entrance. The Permit to Work checkpoints, which are crucial for ensuring the safety of workers entering the space, were simply ticked away without proper consideration.

It is imperative that all necessary safety measures are in place before allowing workers to enter such spaces to prevent any potential accidents or injuries. This has built doubts about the credibility of the Permit to Work when it comes to the levels of toxic gases to pose a serious risk to anyone who entered without proper precautions.
